THE SURGICAL TREATMENT OF A PATIENT WITH SYPHILITIC ANEURYSM OF THE DESCENDING THORACIC AORTA

Abstract

The aortic aneurysm of a specific (syphilitic) etiology in the era of antibiotics is a rare phenomenon that complicates diagnostics of the disease. A lues of cardiovascular system is a late form of syphilis, which is manifested in the 4-5th decade of life, usually after 5-40 years after primary infection. Before emergence of the complaints bound to a compression of surrounding organs the aortic aneurysm can occur asymptomatic. Without surgical intervention the mortality during 1 year can reach 80% because of the high level of rupture of such aneurysms. The exceptional case of syphilitic aneurysm of the descending thoracic aorta is presented in the article. The patient underwent open surgery - aortic aneurysm resection and its replacement with synthetic vascular prosthesis. In connection with the need to protect the internal organs with high aortic clamping the operation was carried out in conditions of cardiopulmonary bypass - distal aortic perfusion via the femoral vessels. However, radical surgery and specific antibiotic treatment do not exclude future manifestations of the disease, even after eradication of Treponema pallidum, which determines the constant need of observation in this group of patients by the appropriate specialists.
