A COMPARATIVE PILOT STUDY OF ANTIPOV POLOSNOGO CHRONIC RHINOSINUSITIS PATIENTS LIVING IN DIFFERENT GEOGRAPHICAL REGIONS OF THE RUSSIAN FEDERATION

Abstract

One of the classifications of chronic rhinosinusitis with nasal polyps (CRSwNP) is based on the type of inflammatory cells which are dominant in polyp tissue. It is generally considered that neutrophilic type of CRSwNP is typical for Mongoloid race and eosinophilic type - for Caucasoid race. Russian Federation is a multinational country situated both in Europe and in Asia. The purpose of the research is to study the correspondence of neutrophils and eosinophils in nasal polyps in different regions of Russia and to juxtapose it with the data on anamnesis and comorbid conditions. Materials and methods 154 samples of polyps from 15 regions of Russia belonging to patients of 12 different ethnic groups of Caucasoid and Mongoloid races were collected. Inflammatory infiltration intensity was measured by an absolute eosinophil and neutrophil count in 10 considered fields of vision with a magnification of 400x and following calculation of the index of their correspondence, which was referred to as eosinophil-neutrophil index (ENI). All patients completed a questionnaire including data about symptoms, ethnicity, comorbidities, allergic reactions and drug hypersensitivity. The results: polyps of eosinophilic type are dominant within the whole territory of Russia (71.7%). No significant difference between various ethnic groups and cell composition of the nasal polyps was revealed. The highest content of eosinophils was detected in samples of the polyps that were collected in several Siberian cities such as Tomsk, Irkutsk and Omsk and in Nizhny Novgorod (central part of Russia). The minimum level of eosinophils was found in Penza and Samara. There was no firm correlation between clinical course characteristics and endotypes of CRSwNP. Statistically relevant differences of ENI measure were found is Tomsk (23.3), Nizhny Novgorod (11.36), Irkutsk (9.14), Omsk (7.02), Penza (2.63), Chelyabinsk (3.93) and Izhevsk (3.73). No significant correlation of ENI with ethnicity, presence of bronchial asthma, NSAIDs-intolerance, sensibility to airborne allergens and presence of reoperations was educed.
