Abstract: Strategic tasks of the State at the modern stage of the country development are demonstrated to include the assurance of high standards of human welfare; development of the system of the National security; the elevation of the competitive ability of goods at the global level; the provision of the balanced space for development and the increase of the efficiency of the state control and supervision. The methodology for the health risk analysis represents the modern tools allowing in combination with traditional methods to solve assigned tasks mostly effectively and completely. The integration of the risk analysis into the solution of strategic tasks of the state social and economic development is in the systemic application of principles and approaches of methodology in the field of lawmaking; improvement of scientific and methodical support for the solutions at all levels, assessment of the efficiency of measures, in the transition to the risk-oriented model of control and supervision activity and monitoring of situation. Approaches to the assessment of the risks evolution, calculation of integral risks, defragmentation of risks and detection of critical points for the transition of risk to the qualitatively new level substantially extend the possibilities of methodology and increase the efficiency of its application in the solution of State tasks on the preservation of health and assurance of the sanitary and epidemiological welfare of population.

PRIORITY RISK FACTORS OF DRINKING WATER AND THE RELATED WITH IT ECONOMICAL LOSS

Abstract: The inadequate water quality of utility and drinking water supply system has been shown to be able to contribute to the additional cases of mortality (approx. 11,0 thousand cases) and morbidity (over 2900 thousand cases) and be the second most important environmental factor. Elevated levels of arsenic, copper, cadmium, organochlorine compounds, nitrates, nitrites and other detrimental impurities can give rise the development of adverse effects on skin and subcutaneous tissue; musculoskeletal system and connective tissue; blood, hematopoietic organs and separate disorders involving the immunity mechanism; urogenital, nervous, endocrine systems, blood circulatory system, digestive organs, both in the population of adults and children. The list of priority risk factors incoming with drinking water and forming negative tendencies of the health state of the population of the Russian Federation changes insignificantly over time. The economic loss due to the negative influence of the priority factors of drinking water pollution on the population’s health accounted for over 50 billion rubles in 2014.

Abstract: The article presents the analysis of the impact of drinking water organochlorine compounds on the population health (Perm region). Results of the carcinogenic and non-carcinogenic health risk assessment for population of the city of Perm and the Perm region under the chronic oral exposure of organochlorine compounds in drinking water are presented. Real concentrations of organochlorine compounds in drinking water were determined to be potent of emerging an unacceptable risk of diseases of the digestive system, endocrine system, hormone system, excretory system, central nervous system and the blood system. According to the results of epidemiological studies there was established the statistically significant relation between the consumption of water containing organochlorine compounds and the development of diseases of the digestive system, endocrine system, central nervous system, and excretory system.

Abstract: In the article there are presented the methodological approaches to the diagnosis, treatment and prevention of diseases associated with the exposure to environmental factors, based on the principles of risk assessment of existing risk factors, confirmed exposure, identification of biomarkers of effect with the use of cellular-molecular, proteomic and nanotechnology, the results of clinical and functional research, analysis of the system relationships, exposure markers with markers for the negative response. Sanitary-hygienic, epidemiological, clinical, functional and laboratory criteria underlying the proposed classification of specialized medical preventive technologies for risk management, allow to make a differential diagnosis of diseases associated with the exposure to environmental factors according to their severity and stage of clinical manifestation, identify target groups ofpopulation to provide them specialized treatment and preventive care, determine the volume, direction, content and form of implementation of curative and preventive measures.

Abstract: In the framework of the solution of tasks of spatial planning and the development of territories of regions of the Russian Federation as well as during the development of general layouts of urban settlements the potential human health risk assessment has been shown to be performed. The use of health risk criteria permits to avoid the underestimation of hazards, to provide safe and favorable living conditions by means of town-planning design; to restrict the harmful impact of anthropogenic and other activities on the environment to the acceptable levels according to the criteria of population health; to use rationally the urban resources to the benefit of living and future generations.

Abstract: Methodical approaches to the integrated evaluation of the exposure and length of service in occupational risk assessment have been proposed. There are: the identification of hazards with a specific assessment of working conditions; the establishment of the occupational dependence of the health disorders in workers on the base of the results of epidemiological studies; quantitative risk assessment, taking into account the work experience factor on the basis of a mathematical model “exposure - work experience - response probability”; the establishment of quantitative risk characteristics, taking into account the dynamics of changes in health status over time, as well as the assessment of an integral risk of accumulation of the body functional disorders on the basis of the risk evolution modeling. When testing an integrated risk assessment approach for the workers occupied in underground mining, there was revealed the occupational dependence both for respiratory diseases (RR = 1.75) and disorders of the circulatory system (RR = up to 1.69). The work experience determination of the occupationally chronic rhinitis and rhinopharyngitis (J30- J31) has been established as well as the predictable probability of the development of hypertension after 5 working years as 0,094, after 10 years -0,179 has been detected. The risk of health problems under exposure to occupational factors becomes unacceptable (R) by 45 years. In this the risk for the occurrence of the circulatory disorders is critical.

ANALYSIS OF THE HEART RATE VARIABILITY IN CARDIAC ABNORMALITIES IN WORKERS EMPLOYED IN OIL PRODUCTION

Abstract: Reduced heart rate variability (HRV) is associated with the elevated mortality rate in patients with cardiovascular disease. Risk factors, in terms of the negative impact on the cardiovascular system of workers of oil production include noise, vibration, the severity of the labor process, climate, lighting, and various chemicals presenting in the working area (the total of various hydrocarbons, including aromatic hydrocarbons, disulfide sulfur). HRV indices were recorded daily by Holter monitoring. Employees with the work experience at oil production from 10 to 20 years were established to have a statistically significant increase in tone of the sympathetic nervous system, characterized by the decline ofpNN50 by 2.2 times and the increase of the vagal tone, characterized by an increase in SDANN by 1.4 times and SDNN - by 1.3 times. A significant dependence of the probability of the increasing of the tone of the sympathetic nervous system from work experience was established. Thus, the impact of hazards of oil-producing enterprise is associated with prognostically unfavorable changes in HRV. These changes may exacerbate hypertension in employees, which requires measures for the primary prevention of cardiovascular diseases.

Abstract: The study of the immune status markers and characteristics of genetic polymorphism in potash mining workers under the impact of combined industrial factors was performed. We showed the inhibition ofphagocytosis and production of serum IgG and IgM, the deficiency of mediators of the immune and endothelial regulation of interleukin-17, VEGF, GAD1 and GAS6. The revealed changes are associated with the negative genetic fixation of key polymorphisms responsible as well for the detoxification of the first and second phase of the neuro-immune regulation, the state of the vascular endothelium and apoptosis, as both due to heterozygous and due to the homozygous variant genotypes of genes of cytochrome (CYP1A1), Sulfotransferase (SULT1A1), toll-like receptor (TLR4), tumor necrosis factor (TNF), endothelial growth factor (VEGFA)

PECULIARITIES OF BEHAVIORAL RISK FACTORS FOR HEALTH IN WORKERS OF INDUSTRIAL ENTERPRISES

Abstract: There is actualized the problem of the needfor the awareness of the peculiarities of the subjective perception of health risks in the particular social group as the first step towards the construction of an effective system for the provision of information about risks. There was noted the presence of the gap between expert and everyday knowledge about risk. On the base of materials of two representative sociological surveys of working population of the Perm region with the use of the author tools of the research, there was performed in-depth analysis of the perception of risks and were shown the features of subjective assessment of health risks related to nutrition, smoking and alcohol consumption in workers of industrial enterprises. The empirical analysis of perception of risks related to peculiarities of individual behavior of employees was pointed to be performed extremely rare. The perception of the industrial workers of the health risks related to behavioral factors was revealed to have a multilevel structure. Under the high level of awareness about behavioral health risk factors, self-protection modes of behavior were established to fail to occur widely among employees. Workers in industrial enterprises were determined to be characterized by the overestimation of the impact of environmental and occupational factors on health along with the underestimating of the significance of individual behavior. There was proved the low efficiency of the linear model for the provision of information about risks. The proposed principles of the construction of risk communications in the field of health, ensuring the successful management of the risk perception by workers in the form of transition from provision of information to communication, that means the reliance on dialogue, the abandonment of unilateral translation of knowledge from the leaders (experts) in favor of regular exchange of information with the aim of shaping of consensus on what risk factors are significant, how they influence on the human health and what should be done to minimize them.

HYGIENIC ASSESSMENT OF WORKING CONDITIONS FOR MEDICAL PERSONNEL IN CLINICAL AND PARACLINICAL DEPARTMENTS OF THE HOSPITAL

Abstract: The aim of the research is the study of working conditions for medical staff and assessment of health risks to under the exposure to chemicals in air at the working zone. A cross-sectional epidemiological study has been carried out. Attestation charts of 706 employees ’workplaces have been analyzed. Hypotheses were checked with the use of x test criteria and Fisher’s exact test. Risk characterization was carried out with the use of hazard quotient (HQ), hazard index (HI) and personal carcinogenic risk (PCR). 99% of the medical staff were established to work in hazardous conditions. In surgical and therapeutic units, about a half of medical staff (80% and 69% respectively) is exposed to the impact of biological factors, and to the impact of chemical substances (50% and 43% respectively). In paraclinical units 82% of medical staff are exposed to biological factors, about 40% - to electromagnetic fields, 48% of the employees work in poor light conditions. The share of the staff working in strenuous conditions, in the surgical units (70%) is higher than in therapeutic and paraclinical units by 39% and 55% respectively (x=239,6; p<0,001) . In therapeutic units the share of work places, where the labor intensity exceeds the allowable limit, is more than in surgical and paraclinical units by 21 and 33%, respectively (x=71,7; p<0,001). Medical staff is under a low risk of general toxic action of chemicals in the workplace air. HQ of ethanol, formaldehyde, chlorine, mercury is less than 0,3. HI to critical organs and systems is less than 0,4. PCR is of acceptable level to the professional environment (CR = 3,6*10).

Abstract: In the process of the exceedance of numerical composition of the groups in preschool educational institutions of the general enrichment orientation over 20 children, there are disturbed conditions for placing children in group cells: the level of acoustic impact as well as the chemical and biological air pollution of the gaming space increases significantly, what leads to the development of chronic stress syndrome in children, the violation of the somatic growth and mental development, decline of the level of adaptive capacity of the cardiovascular, respiratory and autonomic nervous systems, the gain in acute and chronic morbidity rates. There was scientifically justified the index of the optimal occupancy rate per one typical group of Children Educational Organizations (CEO). The presence of not more than 20 children in a group complies with the sanitation requirements of SanPin 2.4.1.3049-13, and the level of the morbidity rate of children is determined minimally by the index of the number of children attending this group.

Abstract: The study of the immune regulation in children living under conditions of exogenous strontium exposure revealed a decrease in phagocytosis, production of serum IgG and IgM, the expression level of CD95+ T- cell receptors, as well as the elevation of specific IgG to strontium and cytokine tumor necrosis factor and the change in the balance of immune osteometabolism regulators RANKL and osteoprotegerin.

DISORDERS OF LIPID AND CARBOHYDRATE METABOLISM IN CHILDREN CONSUMING DRINKING WATER OF A NON-NORMATIVE QUALITY

Abstract: Clinical and laboratory investigation of violations of lipid and carbohydrate metabolism in children consuming drinking water of non-normative quality on content of organochlorine compounds. Children with elevated blood concentrations of chloroform and carbon tetrachloride, endocrine pathology are diagnosed 2.1 times more likely, the relative risk of developing obesity (E66.0) and excess supply (E67.8) is by 2.7 times higher than in the comparison group. There was found a causal relationship between the development of overweight children with a high content of chloroform in blood. Negative effects of organochlorines (chloroform, carbon tetrachloride) manifested by dysfunction of the hypothalamic-adrenal and pituitary-thyroid system, the activation offree radical oxidation and the accumulation ofperoxidation products, overvoltage and resource depletion of antioxidant defense, and violation of neurotransmitter processes was found to be followed by dysregulation offat and carbohydrate exchange. There were identified markers of disorders of lipid and carbohydrate metabolism in the conditions of exposure to chloroform and carbon tetrachloride.

CARDIORESPIRATORY DISORDERS IN PRESCHOOL AGED CHILDREN ASSOCIATED WITH AEROGENIC IMPACT OF BENZENE, PHENOL AND FORMALDEHYDE

Abstract: There was performed an examination of 437 pre-school aged children living in the conditions of the pollution of atmospheric air with benzene, phenol andformaldehyde. Children with the elevated blood content of benzene, phenol andformaldehyde were detected to be diagnosed as having chronic respiratory diseases and asteno-neurotic syndrome by 3 and 1.3 times more often, respectively, in contrast to the reference group. Cardiorespiratory disorders in children with the elevated content of benzene, phenol and formaldehyde are manifested by restrictive and mixed disorders of airway conductance followed by the increase in markers of proliferative cell activity (CA-72-4 and M 20), changes in the electrophysiologicalprocesses in cardiac muscle pronounced by the decreased electric ventricular systole (QT interval) followed by the decline of the content in blood of glutathione-S-transferase, zAMF, nitrogen oxide and the increase in the lipids hydroperoxide level, and creatine phosphokinase activity.

Abstract: There are summarized data on factors affecting on quality and sanitary indices of wheat germs in the process of storage: the number of peroxides, microbiological contamination, bulk weight, angle of natural slip. There was performed an analysis of approaches of stabilization of these indices, there was substantiated the urgency of a comprehensive research on the development of a method for inhibition of oxidative processes and the growth of microorganisms. The treatment of wheat germs before placement in storage with compositions of organic acids - succinic, ascorbic, fumaric acid was found to increase the shelf life of wheat germs to be processedfrom 7 days to 8 weeks. The proposed treatment was revealed to reduce the number ofperoxide value and importance of the bacterial contamination of the germs, at that products of the pressing the germs stabilized by organic acids meet the requirements of the technical documentation, and germ expeller is additionally enriched by compositions of organic acids in an amount of 0,5 - 1,0%.

Abstract: There was made an evaluation of the possibility of the solution of the problem of imbalance of polyunsaturated fatty acids in the diet ration of the population by means of engagement of vegetable sources. There were both analyzed the composition and properties of products of the processing of unoiled domestic plant raw material containing w-3 unsaturated acids and established prospects of their use in technologies of the balancing of polyunsaturated fatty acids in food products.

POND FISHES IN THE IMPROVEMENT OF THE STRUCTURE OF POPULATION’S NUTRITION: HYGIENIC ASPECTS

Abstract: Potential opportunities of fishery resources of internal reservoirs of the country are estimated at 5 million tons that creates prerequisites for the increase in rates of the development of the branch, and, therefore, volumes of the food resources related with the improvement of structure ofpopulation’s nutrition. It should be noted that when processing fish raw materials it is necessary to seek for the achievement of the highest customer value. It is provided with the high quality, goodness, commodity-food indices, food, biological and physiological properties. The goodness is estimated by sanitary and hygienic indices and provides the harmlessness of production for a human body. In the course of own pilot studies fish of internal reservoirs was established to be an excellent source offull-fledged protein, fat and extractive substances which act as the most important factor of the food determining the health of the population. In terms of a mass fraction of protein the meat of pond fishes comes up to the meat of warm-blooded animals. The assessment of a ratio of amino acids in comparison with a standard of FAO shows that in a human body amino acids of proteins of pondfishes are capable to be digested at the level of 60% (U = 0,48-0,73). The content of fat depends on a species offishes and fluctuates from 0,5 to 9,0% (in some cases can reach 30% - in catfish). Accessibility offish fats is very high and amounted for 96-97% at metabolism coefficient as 0,34-0,19. At room temperature cod-liver oil has a liquid consistence and contains more than 80% of unsaturated fatty acids that provides their high accessibility. Muscular tissue offishes contains a full set of vitamins that confirms the high nutrition and biological value offishes. Expansion of the range of fish products in industrial scale will considerably reduce the present deficiency of animal protein in a food ration of the population, will raise a share of useful fats at enrichment by vitamins. The purposeful use in many respects will provide hygienic and physiological norms in nutrition, will create conditions for of the development of the foodstuff and diets correcting and supporting the health of the human.

Abstract: In children under aerotechnogenic impact of phenol, methanol and formaldehyde, these substances are identified in the blood in significantly higher concentrations than in the unexposed group. There was found the activation of the mechanisms of the innate immunity (phagocytic activity) and inhibition of markers of the acquired immunity (reduction of relative indices of the content of T-lymphocytes (CD3+) and their subsets (CD4+- and CD8+-lymphocytes). Out of the markers of neuroendocrine regulation there was proved the significant decline of cortisol. Under the impact of anthropogenic chemical environmental factors there are occurred rearrangement and the transformation of interactions within the neuroendocrine and immune systems, as well as “early”, not manifested yet in the nature of specific nosological forms, but over time having the possibility to lead to the shaping of various pathology changes in the neuroendocrine regulation of the immune system.

BIOCHEMICAL MARKER INDICES OF NEGATIVE IMPACTS IN CHILDREN UNDER THE EXPOSURE TO THE CHLORORGANIC COMPOUNDS WITH DRINKING WATER

Abstract: The exposure to chloroform, dichlorbrommethane, tetrachloromethane and residual chlorine with drinking water at the level of up to 0,008 mg/(kg*day) was shown to stipulate in children the risk for the development of diseases of liver and kidneys, central nervous system, blood and endocrine system which exceeds the acceptable level by 1,5-1,7 times. The marker of exposure to chloroform with drinking water was proved to be the identifiable concentration of chloroform in the blood of children at the level of 0,0002-0,0008 mg/dm. The biomarkers of noncarcinogenic impacts are the indices of the violation of the balance between the oxidative and antioxidative processes (the elevation of the content of hydroperoxides of lipids, malondialdehyde, glutathione peroxidase, Cu/Zn-SOD, antioxidative activity in blood serum, 8-OHdG in urine), the balance of neuromediators of excitation and inhibition in the central nervous system (increase of glutamate and decrease of gamma-aminobutyric acid in the blood serum), enhancement of cytolysis (increase of the ASAT activity in the blood serum) and development of the inflammatory response (elevation of C-reactive protein in the blood serum).

Abstract: In blood of children underwent to the long-term isolated exposure of manganese and stable strontium with drinking water exceeding the acceptable level by up to 1.4 times the elevated concentrations of mentioned metals in blood were shown to be formed in relation to reference level by 2,0-4,4 times. The developing at that mutagenic effect was proved to have the dependence on the level of manganese and stable strontium concentration in blood and is pronounced in the increased rate of cells with cytogenetic anomalies on the background of the enhancement of the proliferative activity, destructive changes in the cell population and oxidative responses at the cell DNA level. The activity of genetic instability and processes of DNA oxidation in the somatic cells in children under the exposure to manganese has more pronounced character in comparison to given indices under the exposure to the stable strontium.

ON THE QUESTION OF THE APPLICATION OF THE PREDICTION OF THE EVOLUTION OF HEALTH RISK IN HYGIENIC ASSESSMENTS

Abstract: In the paper there are described examples of the practical implementation of methodological approaches to the assessment of evolution of the health risk under exposure to a set of environmental factors. In the hygienic assessment of the dynamics of health risks for citizens, residing near a major airport (about 500 takeoffs and landings per day), there were revealed high risks of disorders of the cardiovascular and nervous system. The difference between the risks emerging under the exposure and without the impact of the factors peculiar to the airport was shown to increase with the age significantly, accounting for 5-year period, 1,2% of the exposure; over 20 years - 8-10%, and during the period of 50 years - more than 20%. There were obtained results describing the dynamics of the gain of risks for the population of all ages, the structure of risks in the different periods of the human life. Outputs of evolutionary modeling allowed to identify critical periods of exposure to factors for which the qualitative changes of levels of risk take place. This is important in justifying the order ofpriorities and urgency of sanitary hygienic measures for risk management. The development of methodological approaches lies in the plane of the refinement of mathematical models, improvement of methods for the assessment of the intensity and duration of exposure, taking into account features of the combined impacts of hazard factors of different nature.

Abstract: The methodical peculiarities of methods for the determination of chemical compounds and elements in biological matrices are considered. There are presented examples of practical application of methods for the sample preparation - organic solvent extraction, derivatization, solid-phase extraction, headspace analysis. On the example of the method for the determination the methyl-tret-butyl-ether in blood there is denoted the problem and shown the necessity of the identification of components of biological matrix with the use of the chromato-mass-spectrometry. There are presented examples for the use of the set of factors in order to increase the sensitivity and selectivity in detection of target components.

Abstract: There is considered the complex of methodical modalities for the determination of elements (vanadium, chromium, manganese, nickel, copper, zinc, selenium, strontium, thallium, lead, cadmium, arsenic) in the biological matrices of population for the practical use in the framework of socio-hygienic monitoring and biomonitoring in areas with the high anthropogenic load. Guidelines developed on the basis of mass spectrometry with inductively coupled plasma (ICP-MS) confirmed as Methodological Instructions on Control (In Russian:MUK 4.1.3230-14) allowed to implement a hygienic estimation of the pollution of biological matrices of the adult population residing in the territory exposed to the impact of emission from ferrovanadium enterprises (city of Chusovoy, the Perm region). According to results of the study in the monitoring group in the blood there was established a significant excess of the content vanadium, manganese, nickel, strontium, cadmium and reduced selenium content, in urine - an excess of vanadium and chromium, relatively to the control group.

Abstract: There are presented results of experimental studies on the development of the set of gas chromatographic methods for the determination of acrylonitrile in the atmospheric and expired air based on the sorption of the investigated compound to the Tenax TA sorbent, in combination with optimal conditions of the sample preparation, thermal desorption and the application of capillary gas chromatography. The method allows to implement the determination of acrylonitrile in the samples of air at the level of0,0003 pg in the analyzed volume of sample, with extraction degree of 96,7%. The determination of acrylonitrile in the expired air is possible at the level of0,0002 pg at the volume of air of 1000 cm3, with extraction degree 97,7% selectively, credibly and with high sensitivity, according to the tasks of social and hygienic monitoring. The developed method for the determination of acrylonitrile in blood is based on the extraction of investigated compound from the biological material at pH=2-3 with the use of the equilibrium vapor phase analysis. In combination with optimal conditions fir the sample preparation and the use of capillary gas chromatography, the methods allow to determine acrylonitrile in blood samples at the level of 0,006 mg/dm, with extraction degree of 96,9% and maximum error of 25%.
